In progress: paramedic post-fall pathway

In collaboration with Toronto Paramedic Services and the North Western Toronto Ontario Health Team (NWT OHT), work continues to adapt the RCA’s Older Adult Community Paramedic Post-Fall Rehabilitative Care Pathway for use in the GTA.

Establishing the current state of outpatient rehab to inform system planning

The Network is currently analyzing data it has collected over the past six months in an effort to better understand capacity and demand for outpatient rehabilitative care programs and speciality clinics across the GTA.

Optimizing the inpatient rehab referral process

An inpatient referral small working group has been convened to review the Network’s Rehab/CCC inpatient referral form with a view to optimizing the completion of referral information and reducing Requests for Information (RFIs).
