A new Network initiative is exploring development of guidelines and best practices for inter-organizational transfer of accountability (TOA) to support effective and seamless transitions and improve patient safety. This approach aligns with the MOHLTC-mandated LHIN focus on improving how patients move through the system and Accreditation Canada’s focus on information transfer at care transitions.

As a first step, the Network is working with six Rehab/CCC hospitals* to gather data from October to December 2017 to determine the type and frequency of TOA issues. The Network will collect and analyze the information to identify trends and develop an inter-organizational guideline. Additional Network members from across the GTA have also expressed interest in participating in the initiative.
