The Network’s new population-specific rehab guides are designed to assist Ontario Health Teams (OHTs) in planning and providing rehabilitative care for frail seniors, patients with congestive heart failure and patients with COPD.

The brief guides provide evidence-based information on how rehab can help to achieve positive patient outcomes for individuals in primary care, hospital or at home. They also outline rehab considerations for these populations in the context of COVID-19, as well as recommendations to assist OHTs in planning rehabilitative care.

Rehabilitative care is an important component of a population health approach, contributing to better patient outcomes and a more efficient health care system. The GTA Rehab Network is available to help OHTs plan how to integrate rehab services into the care provided to their priority populations.
