The GTA Rehab Network is proud to be one of 26 organizations across Canada invited to partner with Healthcare Excellence Canada and others as part of the Enabling Aging in Place collaborative.  The Network will use this funding to support project planning for community paramedic post-fall pathway implementation in the North Western Toronto OHT, in collaboration with Toronto Paramedic Services. This initiative aims to assist older adults who have fallen and not been transported to hospital, but who may benefit from referrals to geriatric rehabilitative care services or other programs to help prevent future falls.

We are so grateful for this funding and look forward to collaborating with others on this important initiative.
