The GTA Rehab Network has completed development of a standardized referral form to support direct access from emergency departments (EDs) to inpatient rehabilitation. The form was created with input from five EDs and seven inpatient rehab programs, ensuring it reflects the needs of both sending and receiving sites.

To validate the tool, the Network conducted a six-week pilot with Scarborough Health Network, Mount Sinai Hospital, Providence Healthcare, and Hennick Bridgepoint Hospital. Feedback from the pilot informed refinements to the form and confirmed its value in streamlining referrals and supporting smoother transitions in care.

The new referral form went live in the Resource Matching and Referral (RM&R) application on September 17th and will be used to support Ontario Health Toronto’s expansion of the ED-to-inpatient rehab pathway. This new process aims to reduce delays, improve patient flow, and enhance timely access to rehabilitative care.
