To support direct access to inpatient rehabilitation from emergency departments, the GTA Rehab Network has developed a draft referral form and process aimed at streamlining this pathway.

The draft tools are currently being tested through a pilot with a small group of participating ED and inpatient rehab sites. This work builds on ongoing efforts to improve transitions in care and reduce unnecessary delays for patients who may benefit from early rehab intervention.
