Paramedic Post-Fall Pathway pilot complete in North Western Toronto OHT

The North Western Toronto Ontario Health Team has completed its Community Paramedic Post-Fall Pathway pilot, which ran from July 2024 to June 2025 and included three Plan-Do-Study-Act (PDSA) cycles. The initiative was designed to strengthen connections between community paramedics, primary care, rehabilitation, and other supports, ensuring individuals who experience a fall are quickly linked to the right services.
Throughout the three PDSA cycles, the pathway demonstrated steady progress. Referrals broadened to include community rehab clinics and fall prevention programs, in addition to specialized geriatric services and home and community care. Community paramedics also reported improvements in the quality and consistency of their assessments, as well as better sharing of this information with primary care teams and other providers.
Both clients and providers shared positive feedback, noting that the pathway has improved coordination across sectors and helped ensure more timely, appropriate follow-up after a fall. The pilot’s results highlight the value of a standardized, collaborative approach to post-fall care and its potential to strengthen community supports for older adults.
Looking ahead, the GTA Rehab Network will be engaging with other Ontario Health Teams to explore opportunities to spread this model more broadly. This initiative also directly supports the Network’s new strategic pillar of building healthier communities, with a focus on influencing a health system grounded in upstream prevention, equity, and recognition of rehabilitation as essential to all care.
