Updated Rehab/CCC Referral Form and Toolkit

To improve patient flow from acute care to rehab and complex continuing care (CCC), the Network has reviewed and refined the tools used in the inpatient rehab referral process.
Based on their work, an updated toolkit of resources designed to streamline and standardize referrals is now available. Key updates include:
- A revised Reference Guide for Completing Referrals
- An updated Referral Process Guideline for Bedded Rehab
- A refreshed Inpatient Rehab/CCC Referral Tip Sheet
- A newly redesigned Inpatient Rehab/CCC Referral Form, available in both paper-based and e-referral formats
On February 4, 2025, the Network hosted a well-attended webinar to share the changes and answer questions about the updated referral form and toolkit.
