On September 17th, changes were implemented in the Resource Matching and Referral (RM&R) application to strengthen the quality and consistency of information included in inpatient rehab/CCC referrals. These updates are based on feedback gathered through a survey of referral-receiving sites and additional input from members.

New mandatory fields
Following the launch of the updated referral form in February 2025, sites reported overall improvement in the quality of referrals submitted. However, common issues included incomplete or missing information and contradictory details across sections. To address these gaps, the following fields will now be mandatory in RM&R:

  • Bed offer contact name and number
  • Height and weight
  • Size of wounds


New decline reason
The GTA Rehab Network’s Referral Tip Sheet states that when there is no response after three business days to a Request for Information (RFI) for a referral submitted to Rehab/CCC, the referral should be declined and removed from the RMR dashboard. To align with this process, a new Decline reason has been added: No response to RFI within 3 business days.
