The GTA Rehab Network continues to work with members and partners to better understand system capacity and opportunities to support patients with tracheostomies as they transition from acute care to rehabilitation and complex continuing care (CCC).

Following recommendations from the tracheostomy working group, additional information has been collected to help inform the direction of the initiative. This included a refreshed analysis of Resource Matching and Referral (RM&R) data examining open and closed cases, as well as wait times for patients transitioning from acute care to rehab/CCC. The analysis was completed in early 2026.

The Network also conducted a questionnaire with acute care and rehabilitation member organizations to identify system barriers and explore potential solutions. Eleven acute care sites and seven rehabilitation sites participated in the survey, which was completed in March 2026.

Findings from both the data analysis and member questionnaire were reviewed with the tracheostomy working group and Ontario Health Toronto in June. These discussions are helping to inform ongoing exploration of potential approaches to better support patients with tracheostomies and their transitions across the continuum of care.
