Community Paramedic Post-Fall Pathway expansion underway
Date: June 22, 2026
The GTA Rehab Network continues to support the spread and scale of the Community Paramedic Post-Fall Pathway (PFP) across Ontario Health Teams (OHTs) and regional paramedic services throughout the Toronto and Central regions.
On June 1, 2026, the Network launched a PFP pilot in the West Toronto OHT, and a further launch is planned in the North Toronto OHT for September. In addition, the Network continues to collaborate with OHTs in the Central region (Connected Care Halton, Burlington, Northern York South Simcoe, Western York Region OHT) to establish their readiness for pathway implementation in the coming months.
Developed by the Rehabilitative Care Alliance (RCA), the Community Paramedic PFP focuses on older adults who call 911 for a fall, or report a fall to paramedics, but who are not transported to hospital. Community paramedics assess fall risk factors and connect individuals to appropriate rehabilitation, fall prevention, and community-based services. Following the Network’s successful pilot completed in collaboration with Toronto Community Paramedic Services and the North Western Toronto OHT, the Network continues to engage with Ontario Health Teams, regional paramedic services, and Ontario Health atHome partners to support implementation in additional communities.
The pathway has also gained broader visibility through presentations at the GTA Rehab Network’s Best Practices Day 2026 Conference and the provincial Community Paramedicine Knowledge Exchange Main Expo 2026, helping to share lessons learned and support adoption in other communities.
